SUMMARY DESCRIPTION

The Certified Substance Abuse Counselor (CSAC) provides counseling and support to individuals and families experiencing addictions. This may include confidential individual, family or group counseling about the causes and effects of addictions support for families dealing with addictions and/or referrals to treatment for individuals requesting this opportunity.

REPRESENTATIVE DUTIES

The following duties are typical for the Certified Substance Abuse Counselor. Incumbents may not perform all of the listed duties and/or may be required to perform additional or different duties from those set forth below to address business needs and changing business practices.

  • Provide drug and alcohol counseling and support to individuals, families and groups in the community:
  • Provide counseling to individuals regarding the use and effects of alcohol and drugs
  • Provide counseling to pregnant women about the effects of drugs and alcohol on the fetus, as requested
  • Accept referrals from individuals, families and agencies in the community
  • Provide group counseling as requested
  • Ensure legislation, standards and guidelines are being followed
  • Maintain strict confidentiality guidelines regarding all clients, conversations and referrals
  • Provide community education and support, as requested
  • Provide community education and support programs, as requested
  • Access and collect relevant information and resources
  • Distribute relevant information
  • Attend QI work group meetings
  • Make visits and presentations to various groups and organizations including school aged children
  • Access or develop promotional materials and information under the oversight of the Lead PCP and/or the Medical Director
  • Provide Treatment Referrals, as required
  • Assist individuals to identify their need for treatment programs
  • Liaise with interdisciplinary team members
  • Establish and meet programmatic performance metrics
  • Liaise with community agencies and supports
  • Ensure ongoing contact with agencies including hospitals, FQHCs, Social Services Agencies, Agencies practicing behavioral health and substance abuse services.
  • Conduct case conferencing with other agencies, as appropriate
  • Manage and administer the alcohol and drug program
  • Maintain confidential written records of client interactions in compliance with HIPAA regulations
  • Create and maintain current and accurate records of the numbers of clients served and the issues
  • Create and maintain historical data for community contacts, presentations and workshops
  • Research and develop future presentations and programs
  • Maintain a resource library of information about addictions, prevention and other related topics
  • Perform other related duties as required

EDUCATION, EXPERIENCE & QUALIFICATION GUIDELINES

Any combination of education and experience that would likely provide the required knowledge and abilities is qualifying.

  • Essential course certification in alcohol and drug from a certified college
  • 1-3 years experience in addiction counseling
  • 1 year in a Patient Centered Medical Home, clinical setting or knowledge of the Patient Centered Medical Home Initiative (PCMH)
  • Experience with Electronic Medical Records
  • PREFERRED - Masters Degree relevant to addiction counseling

  • Able to communicate and relate well with physicians, clinical support staff and other HKH employees.
  • Able to provide innovative input into the development of the clinic and its processes.
  • Maintains appropriate licensure and certification including meeting and/or exceeding CME requirements.
  • Efficient, organized and accurate.
