What You'll Do
Manage a panel of patients, ensuring they receive the care and resources they need.
Identify and coordinate community resources that support patient health and well-being.
Educate patients on managing chronic conditions, improving health literacy, and promoting medication adherence.
Perform medication management, including reconciliation, refills, and identifying potential concerns.
Coordinate essential services such as home health, durable medical equipment (DME), home infusion, and other critical patient needs.
Assist with referrals and appointment scheduling to ensure timely access to care.
Build trust with patients through clear communication and active problem-solving.
What We're Looking For
✔ Active Multi-State/Compact LPN or LVN License (NLC)
✔ Strong technical skills - Proficiency with Microsoft Office Suite and electronic documentation.
✔ Excellent communication - Clear written and verbal skills to educate and support patients effectively.

Bonus Points If You Have:
➕ Experience with Epic or other EHR systems
➕ Bilingual skills
➕ Additional state LPN licensure

Physical Requirements
Ability to sit for extended periods while working remotely.
Strong listening and speaking skills to engage with patients by phone.
Ability to take thorough, real-time notes during patient interactions.
Compensation & Benefits
Pay: $21/hr - $28/hr + overtime opportunities
