Position Overview

The Per Diem Nurse Practitioner reports to the Clinical Operations Director or Supervising Physician. This flexible, as-needed is a member of the Care at Home team, providing equitable, value-based medical care and social support in patients’ homes or through telehealth. The primary goal is to prevent adverse health complications, manage disease progression, and reduce unnecessary hospitalizations by delivering timely clinical interventions.

Responsibilities

  • Conduct comprehensive and follow-up patient assessments through in-home or virtual telehealth visits, including Annual Wellness Visits (AWV), Health Risk Assessments (HRA), post-hospital visits, and acute care visits.
  • Embrace value-based care focused on reducing healthcare costs and improving patient outcomes through home-based interventions.
  • Identify and assess acute and chronic diagnoses, coordinating care management as needed.
  • Collaborate with a multidisciplinary team to develop personalized treatment plans, including prescribing medications, therapies, and lifestyle modifications.
  • Communicate complex medical information with compassion and clarity to patients and caregivers.
  • Promote illness prevention, health promotion, and support for chronic disease management.
  • Document all clinical activities in the electronic medical record system in accordance with policies and procedures.
  • Stay current with medical advancements and engage in professional development.
  • Assist patients in proactively managing their health, including medication adherence, follow-up appointments, and mental health needs.
  • Adhere to professional ethics, federal and state regulations, and HIPAA guidelines.
  • Contribute to quality improvement initiatives and educational programs.

Qualifications

  • Graduate of an accredited Nurse Practitioner program.
  • Board Certification from ANCC or AANP.
  • Prescriptive authority in the state of hire, including DEA registration.
  • Valid NPI and American Heart Association BLS certification.
  • At least 3-5 years of clinical experience, preferably in primary care, hospice, or outpatient settings.
  • Active, unrestricted NP license in the state of hire (eNLC license preferred).
  • Valid driver’s license, auto insurance, and reliable transportation.
  • Experience with EMRs and Microsoft Office Suite.
  • Strong assessment, analytical, and critical-thinking skills.
  • Effective problem-solving and decision-making abilities.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and adapt to a flexible schedule.

Preferred Qualifications

  • Knowledge of HCC documentation, ICD-10 Coding, Health Risk Assessments, HEDIS, and STAR quality measures.
  • Experience in geriatric care and advanced care planning.
  • Bilingual proficiency is a plus.

Schedule

This is a per diem position offering flexible scheduling based on organizational needs and provider availability. Shifts may include weekdays, weekends, and holidays, with potential on-call responsibilities.

Location

Field with travel to Family members’ homes and clinical settings.

The working environment and physical requirements of the job include:

This position requires in-home, assisted living, and independent-living community based work. The job requires frequent travel for patient visits in all types of weather conditions. Work may be performed in settings with conditioned air, artificial light, and an open workspace.

In this position you will need an ability to travel frequently by car and/or public transportation, the ability to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting or standing for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Lifting of up to 50 lbs. occasionally may be required. Good manual dexterity for the use of common office equipment such as computer terminals, calculator, copiers, and FAX machines. Good reasoning ability is important. Able to understand and utilize management reports, memos, and other documents to conduct business.
