IDR Healthcare is seeking a local contract nurse RN Case Management for a local contract nursing job in Dothan, Alabama.

Position: Registered Nurse (RN) – Hybrid Case Manager
Location: Dothan, AL
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (local time)
Model: Hybrid – 4 days in the field, 1 day remote
Assignment Type: Permanent (option to start as 13-week contract with potential to extend or convert)

Join a mission-driven, value-based care team delivering comprehensive services to patients with chronic and complex conditions. This RN role supports in-home and virtual care coordination, focusing on preventative health screenings, education, and long-term care planning.

? Responsibilities

• Conduct annual wellness exams, health screenings, and post-discharge follow-ups
• Provide patient education on available health programs and disease prevention
• Assess and document diagnoses, medications, allergies, and care plans
• Complete documentation using Athena and Salesforce EMRs
• Participate in virtual interdisciplinary team meetings
• Support care transitions and help patients access community resources

Visit Volume:

• 4–8 visits/day (blend of assessments and follow-ups)
• New patients: 90 minutes
• Follow-ups: 30–60 minutes

✅ Requirements

• Active RN license in the state of Alabama (or compact)
• Minimum 2 years of RN experience required
• Strongly preferred: Experience in Case Management, Home Health, Palliative, Hospice, Dialysis, Nephrology, or Telehealth
• Must be comfortable using EMRs, Salesforce, and Microsoft Office (training provided)
• Reliable phone and internet connection
• Valid driver’s license and dependable transportation
• BLS certification

? Training & Equipment

• Laptop provided
• 2 weeks virtual training (paid)
• 2.5 days of in-person training in Brentwood, TN
  • Flights, hotel, and meals covered (up to $50/day)
  • Laptop provided during onboarding

Hybrid role: Must be comfortable with both in-home and virtual care

Must be based in or near Dothan, AL
